couple questions from noob

My girl has endometriosis. She has been told that surgery is the only way to correct the symptoms. She has no medical insurance, so this isn't an option. We live in AZ, so no medical mj laws. Do you have to live in state to get a card? If not, do you just contact a dr for an evaluation and which states would be an option. Thanks in advance. herbavoreAZ
